Recent phase III trial data have expanded first-line treatment options for patients with locally advanced or metastatic triple-negative breast cancer (TNBC) who are not candidates for immune checkpoint inhibitor therapy. In this activity, learners will examine the clinical rationale for targeting TROP2, key differences among TROP2-directed antibody–drug conjugates, and efficacy and safety findings from the ASCENT-03 and TROPION-Breast02 phase III trials. Learners will also explore how PD-L1 status, disease burden, comorbidities, toxicity profiles, and patient preferences inform individualized treatment selection, safety monitoring, and integration of TROP2-directed therapies into the frontline setting.

Pre Assessment

Assess your current knowledge and clinical approach before beginning your text module.
1.

How many people with breast cancer do you provide care for in a typical month?

2.

For those who practice in academic or community settings, please indicate your practice setting:

3.

A 62-year-old woman is newly diagnosed with metastatic TNBC involving the liver and bone. Her tumor is HER2 IHC 0 and PD-L1 combined positive score (CPS) 4. Germline BRCA1/2 testing is negative. She has well-controlled hypertension and type 2 diabetes managed with oral medications. ECOG PS is 1, and she has no history of interstitial lung disease/pneumonitis, significant ocular disease, chronic diarrhea, or clinically significant neutropenia. She has not received prior systemic therapy for metastatic disease.

Which TROP2-directed ADCs are currently FDA-indicated first-line treatment options for this patient?

4.

A patient with newly diagnosed metastatic TNBC who is not a candidate for immunotherapy is considering first-line Dato-DXd. She asks what the phase III evidence shows about the likelihood of tumor response and whether treatment can delay disease progression or improve survival compared with chemotherapy.

Which statement best summarizes the TROPION-Breast02 efficacy results to discuss with this patient?
